
Behavioral Health Care Coordinator
The Behavioral Health Care Coordinator (BHCC) works in collaboration and continuous partnership with chronically ill or high-risk mental health patients and their family/caregiver(s), clinic/hospital/specialty providers and staff, and community resources in a team approach. As a BHCC, you will be responsible for performing a variety of clerical functions to ensure an efficient operation and to enhance patient experience. This role would be responsible for scheduling initial and follow-up visits with a Behavioral Health provider, reminder appointment calls, insurance eligibility, Behavioral Health referrals, patient check-in and check-out and other related duties.
MAT Navigators (Medication - Assisted Treatment)
The MAT Navigator works in collaboration and continuous partnership with chronically ill or “high-risk” patients and their family/caregiver(s), clinic/hospital/specialty providers and staff, and community resources in a team approach. The MAT Navigator plays a pivitol role in supporting the Addiction Medicine provider with a primary focus on outreach services. As a MAT Navigator, you will facilitate SUD services, raise Substance Abuse awareness to school and the under-serve communities, increase comprehension through culturally and linguistically appropriate MAT education, Develop and strengthen community partnership, and Serve as the contact point, advocate, and informational resource for MAT services and expansion.
